Thank you for your interest in Ultimate Health Plans. We offer Medicare Advantage Plans for all individuals with Medicare, including those with chronic health conditions and Medicaid, and proudly serve countries of Citrus, Hernando, Hillsborough, Indian River, Lake, Manatee, Marion, Orange, Osceola, Pasco, Pinellas, Polk, Sarasota, Seminole, St. Lucie, and Sumter.

Are you ready to join Ultimate Health Plans? Check the list of requirements below to see if you're eligible to join.

Generally, you are eligible to join Ultimate Health Plans if:
  • You have Medicare Parts A and B
  • You are lawfully present in the United States and are not incarcerated
  • You are applying during an approved enrollment period
  • If you’re applying outside of this time, please call us to find out if you’re eligible for a special election period. We’re here to help
  • If you’re enrolling into one of our Chronic Special Needs Plans (C-SNP), you must have one of the following chronic conditions: Diabetes, Cardiovascular Disease, Chronic Lung Disorder, or Heart Failure
  • If you're enrolling into one of our Dual Special Needs Plans (D-SNP), you must be actively enrolled in the State of Florida Medicaid program

How to Enroll
  • To enroll in one of our Chronic Special Needs Plans (C-SNP), please call us at 1-844-891-2121 (TTY 711) or complete the Enrollment Form below and mail it to us. Online enrollment is not available for C-SNP plans.

  • You can also enroll online through the Centers for Medicare and Medicaid Services (CMS) website. Follow the instructions on the site to complete your enrollment. CMS will send us your application electronically.
