We have implemented our public health protocols in response to the Coronavirus (COVID-19). To protect the health of our members, our offices have temporarily restricted in-office visits.

We are available Monday through Sunday from 8:00am - 8:00pm EST. During certain times of the year we may use alternative technologies to answer your call on weekends and Federal holidays.

  • EDI: For faster payment, we accept claims electronically through Change Healthcare (877-363-3666). Our ID# is 77022.
  • Paper Claims: We only accept CMS 1500 “red form” for claims and encounters (no handwritten or replicated forms please). Send paper claims to:
